HOW TO BUILD A CHIMNEY

I NEED TO ERECT AN EXTERNAL CHIMNEY. I AM PUTTING A WOOD FURNACE IN THE BASEMENT. I AM GOING TO GO OUT THE BASEMENT WINDOW OPENING WHICH IS AT GROUND LEVEL,AND INTO THE CHIMNEY WE HAVE SOME VERY COLD WINTERS HERE SO THE FROST LEVEL IS A FACTOR.HOW DEEP SHOULD I GO DOWN. HOW THICK SHOULD THE FOOTER BE. HOW TIGHT TO THE HOUSE SHOULD IT BE, AND DOES IT GET ATTACHED TO THE HOUSE AS IT GOES UP.I INTEND ON USING THE 8" BLOCK AND LINERS.DOES IT HAVE TO BE PAST THE PEAK OF THE HOUSE.IS THERE A LINER MADE WITH A HOLE IN IT OR DO I HAVE TO CUT IT.HOW ABOUT THE BLOCKS IS THERE ONE THE SAME WAY,OR DO I HAVE TO CUT IT, IF SO HOW. HOW DO I PUT THE CLEAN OUT DOOR IN IT.AND WHERE. I CAN DO THE WORK WITH THE RIGHT ADVICE. THANKS DAVE.
