Cracked/uneven driveway

We have an 8 year old home in Georgia; the driveway, which is concrete and on a slope, has slowly cracked and become uneven due to apparent erosion underneath the driveway. The driveway was built without any re-bar in it, and it appears to have "sunk" as far as it will go. I'd like to either re-surface it, with something like Perma-Crete, or completely tear it out and replace it with concrete, reinforced with re-bar. The complete replacement cost is about $5,000; I have no idea what the re-surfacing cost would be. My question is, would you recommend mud-jacking and/or resurfacing with Perma-Crete? Is that a viable solution to this problem, or would it be a waste of money?
