The fact that you are using a water softner indicates that the water source itself should be suspect. You might check the web or yellow pages for a water testing facility, and take water samples from a source before the softner, before the water heater, after the water heater, and cold water after the softener. Use new plastic baby bottles to take at least 4 ounce samples (or whatever the lab recommends.) The lab should be able to tell you the source of the problem.

Many water softner companies might do this for you for free, but you pay the price of having to deal with sales people. I prefer to just pay a lab and be done.

Couldn't tell you off the top of my head, but it could also be coming from the water heater itself...
