I am not familiar with precisly how hot your fireplace gets or what color it is. However, there is "grill paint" available at most hardware stores. Unfortunatly it is mostly found in black. This paint is designed for gas and charcol grills so it will withstand very high temeratures. I would also look at auto parts stores or an auto paint dealer. I think there are automotive paints designed to paint engine components such as exhaust manifolds which reach high temperatures. One advantage to the auto paints would be a greater selection of color if you can find some to take the heat.

I would try lightly sanding the rusted areas and then painting. In this aplication I would NOT use a primer unless it was specifically rated for the high temperatures.
