First of all how deep was the concrete and how deep were the anchor bolts installed?

The concrete should be set at least 3-4 feet thick (deep) either in sonotubes or in a cylindrical hole dug with a post digger or auger.

The bolt should be as (long) deep as the hole if possible.

If you only installed a few inches of concrete and set a very short bolt, this could be your problem..

Cocret should set at least a week and preferrably about21 days to reach about an 80% cure. Putting severe pressure on it before then is asking for troube and failure.
