Loose Shower Floor Tiles

We have a three year old house. The shower floor is 4x4 ceramic tile. The tile was laid on mortar on a sand bed (required for proper drainage, according to the installer)on top of a plastic pen. Normal grout was used. After a while, the tiles began to come loose, allowing water to get under them. The floor was redone the same way but drying time for each step was increased. Still, the problem eventually recurred. I have tried using substitutes for grout, such as silicone tub and tile sealer but they always come out after a few showers. Does anyone know a solution to this problem? Any suggestions would be appreciated.
